Your first visit is not only about our team getting to know you, but also about you getting to know our team. We are committed to providing exceptional dental care in a compassionate and gentle manner. In the time we spend together, we encourage you to ask questions, express your concerns and share anything you would like us to know to help us make your care as individualized as possible.
Please complete your New Patient Forms prior to your visit.
In order to serve you better, please bring your PHOTO ID, a list of CURRENT MEDICATIONS you are taking, and your DENTAL INSURANCE CARD (if you have dental insurance).
We recognize the value of your time. Unless there is an emergency situation, you can expect us to be on time. If you need to Reschedule or Cancel your appointment, please provide at least 48 BUSINESS HOURS notice to avoid a Cancellation Fee. Our regular business hours are Monday – Thursday from 7AM – 5PM.
Please RESERVE approximately 1-2 HOURS for your first visit. At this visit you can expect:
Dr. Thurman REQUIRES diagnostic x-rays. We will take all necessary x-rays during your first visit. If you have any recent x-rays you would like us to consider using, it is your responsibility to have them emailed to smile@dentalelements.com PRIOR to your appointment.
